Pleural mesothelioma

Mesothelioma is a malignant tumor of the mesothelium, envelope consists of two layers between which is a liquid lubricant. The mesothelium surrounds many organs: it called for the lung pleura, peritoneum for the digestive organs, pericardium to the heart.

It is almost always secondary to the inhalation of asbestos dust (this is known since the 60s) and possibly some refractory ceramic fibers. Microfibers inhaled asbestos will stay deep in the respiratory tree in the cells, these high strength fibers migrate to the pleura where mechanical irritation of the pleura, the lesion base is primarily physical, not chemical. The period between exposure and disease, often by several decades but can be shorter in some cases. Smoking increases the risk of lung injury but does not affect the risk of developing mesothelioma.
The signs of the disease are not very evocative and often late: Chest pain, neuralgia in the arm, back pain in the shoulder blade. Therefore considered rather banal More evocative: pleural effusion (fluid between the often bloody layers of the pleura) with dyspnea (breathlessness),
The chest X-ray is rarely useful except in advanced stages. Microscopic examination of the aspirate in an effusion of malignant cells can be found CT and MRI examinations are the most useful and diagnosis will be confirmed by a biopsy of the pleura or by thoracoscopy
Treatment: early surgery can produce results, then radiation therapy can slow the progression, and finally chemotherapy will be only palliative. The prognosis is generally awful (one year survival in many cases only).
Prevention is essential: removal of sprayed asbestos, protection of workers and their families (wife cleans clothes for example) Asbestos is banned in France since 1997, and in many western countries (but there are other countries where everything needs to be done ...)
Occupations at risk are numerous. Those who pay the highest price has been, is and will be (the maximum number of cases is expected between 2010 and 2020):

Mesothelium

The mesothelium has two layers, one surrounding the body itself, and the other forms a sac around it completely. A small amount of fluid is normally produced between these two layers, lubricating the movement of organs protected. When normal cells of the mesothelium lose control and spread rapidly, they produce mesothelioma. The form of mesothelioma is the most common pleural mesothelioma. It occurs in the pleura, the lining of the lungs. Other forms are peritoneal mesothelioma, reaching the peritoneum lining the abdominal cavity and pericardial mesothelioma, the pericardium.

Development first on the parietal pleura of No demonstration that the pleural plaques are associated with an increased risk of mesothelioma. Predominant form epithelioid; mixed forms and sarcomatoid rare. Immunohistochemical staining to distinguish: epithelioid mesothelioma and adenocarcinoma: marker associated with mesothelioma: calretinin, podoplanine, WT1, cytokeratin 5 / 6. Historical of cancer

An article published by Wagner et al in 1960 established for the first time that mesothelioma is a disease resulting from exposure to asbestos in the form of crocidolite12. The article lists more than 30 cases of patients who had mesothelioma in South Africa including temporary exhibitions and cases involving minors.
In 1962 Dr McNulty reported the first case of malignant mesothelioma diagnosed in Australia in a worker exposed to amiante13. The worker had worked in the mill at the asbestos mine in Wittenoom from 1948 to 1950.
In the town of Wittenoom, packaging waste of asbestos from the mine were recycled in the coating of playgrounds and playground
In 1965 it was established in an article in the British Journal of Occupational Medicine that people who had lived in the neighborhood of factories and asbestos mines, but without work, had contracted mesothelioma.

Treatment of cancer

The mesothelioma treatment by conventional therapies has not proved effective and patients have a median survival of 6 to 12 months after diagnosis. The degree of malignancy of the tumor depends on several factors including the total mesothelial surfaces of the pleural cavity which promotes the importance of local metastasis via exfoliated cells and the invasion of underlying tissue and other organs in the pleural cavity. Another factor involved is the extremely long latency period between exposure to asbestos and the development of the disease. Surgery There are two types of surgery. The partial pleurectomy lightest and extended pleurotectomie that involves removing the pleura and lung block. In some cases, surgery may significantly increase the survival expectancy. Radiotherapy Chemotherapy In February 2004, the Food and Drug Administration approved the use of pemetrexed for the treatment of malignant pleural mesothelioma. The use of this drug is legal in France since 2005. Raltitrexed in turn is being recorded in France and should get his MA in early 2011. The addition of bevacizumab to chemotherapy (gemcitabine + cisplatin) appears to be décevant10. A European trial (IFCT-0701 MAPS) is ongoing to assess the value of bevacizumb in combination with standard chemotherapy (pemetrexed + cisplatin). Other protocols have been tried with moderate efficacy, survival compared to more conventional treatment not exceeding a few months11. Immunotherapy Treatment protocols involving treatment with immunotherapy have yielded variable results. For example, intrapleural inoculation of Bacillus Calmette-Guerin (BCG) in an attempt to boost the immune response, proved of no benefit to the patient (while it can improve the patients of bladder cancer). It has been shown in vitro that mesothelioma cells were destroyed by lymphocytes following activation by interleukin-2 (IL-2), but patients undergoing this particular therapy have experienced major side effects. Indeed, this clinical trial was halted because of unacceptably high levels of toxicity of IL-2 and severity of side effects such as fever and cachexia. However, other trials using interferon alpha have been encouraging with 20% of patients with greater than 50% reduction in tumor mass combined with minimal side effects. Tags : mesothelioma treatment
